¤ photocopy of front page of U.S. Passport of Child's mother, ____________________________ (Mother's Name), a U.S. citizen;

¤ photocopy of front page of U.S. Passport of Child's father, _______________________ (Father's Name), a U.S. citizen;

¤ photocopy of front page of Child's passport issued by ___________ (name of foreign country) in the name of __________________ (Child's Foreign Name), Child's former name;

¤ photocopy of certified copy of Marriage Certificate of Child's Parents (if applicable);

¤ photocopy of certified copy of Divorce Decree of Child's Mother (Father) (if applicable);

¤ a check in the amount of $200 payable to the U.S. Immigration and Naturalization Service.

Copies of documents submitted are exact photocopies of unaltered original documents, and I understand I may be required to submit original documents to an Immigration or Consular official at a later date.
